First published in London by Hutchinson in September 2006 as a hardback (ISBN 0091800951). Simon & Schuster issued the American edition in New York the same year. Hutchinson also produced a trade paperback (ISBN 0091801257) alongside the hardback, plus two special issues: a Waterstone's edition (ISBN 0091795443) and a leatherbound edition (ISBN 9780091795498). Check the ISBN on the copyright page and jacket to tell which issue a copy is; the standard cased issue is the trade first. The Arrow paperback followed in 2007.
