Farrar, Straus and Giroux in New York published the true first in 2001. Library records say it was first published in Great Britain by Fourth Estate in 2001. Fourth Estate's large format paperback (ISBN 9781841157894, 568 pages) is dated 2001. The Fourth Estate hardback (ISBN 9781841156729) is the format collectors usually call the UK first; Open Library and one dealer date it 2001, but most dealers date it 2002, so which UK issue came first is not settled. The hardback jacket was designed by Julian Humphries with a front illustration by Sarah White. The 653 page paperback of 2002 (ISBN 9781841156736) and the 2009 numbered edition of 2,000 copies are later. No print run or issue points were found.
